April 15 from 8am - 5pm AND April 29 from 8am - 12pm

Prerequisites and required documents:
- ADA Graduate Dental Assistant OR proof of completion of POI and ICP courses.
- Medical History form - please print form, fill out, scan and email back to ejbrown@ccsnh.edu
- Informed Consent form
Required by student if COVID protocols allow:
- Treat 5 paitents under the supervision of a NH dentist between clinical sessions.
Required PPE: Lab coat, safety glasses, 2 face masks, N95 mask
