• ADA Graduate Dental Assistant OR proof of completion of POI and ICP courses.
• Medical History Form - please print form, fill out, scan and email back to MMcManus@ccsnh.edu
• Informed Consent Form

**You will NOT be officially registered and a seat will not be reserved until ALL necessary paperwork as well as payment has been submitted.

Required by student:

• Treat 5 paitents under the supervision of a NH dentist between clinical sessions.

Required PPE: Lab coat, safety glasses, 2 face masks, N95 mask
